Current Patients

Updating your contact information, insurance information, or health information

If you are a current patient, we kindly ask that you keep us informed if you have any changes to your address, phone number, insurance information, or changes to your vision or overall health. What you should bring to each exam:

  • Medical and vision insurance cards
  • Your present glasses and contact lenses
  • Sunglasses and other protective eyewear
  • List of Changes in medical conditions and eye-related problems
  • List of medications you currently take
  • List of allergies
  • A referral, if your insurance company requires this

New Patients

Making your First Appointment

Kindly call our office at 724-758-6338 (Ellwood City) or 724-452-6675 (Zelienople). Our patient coordinators will gather information to determine your current vision and medical needs, your insurance details, your address/phone/contact information, and your preferred dates/times for your visit.

What to Expect During Your First Visit

Our Doctors prefer an individualized approach to caring for patients with the goal of offering the best eye health care.

  • They will review your medical history (from your new patient form) as well as any medications you are currently taking.
  • Our technicians will start your examination, performing necessary tests. The tests performed may vary depending on your particular needs, prior medical history, or responses.
  • Your Doctor will be given all test results to go over with you and will continue your examination. Examinations may include dilation of the pupils, fundus photography, visual field testing, corneal topography, glaucoma testing, blood pressure, visual acuities, refraction for determination of glasses prescription, nerve fiber analysis, and color vision testing, to name a few. All of the information you give us and that we find with testing will determine exactly what we need to do to professionally and appropriately customize your care. Depending on these results, additional visits may be necessary.
  • Prescriptions for glaucoma or other medical problems can also be written at your visit, if required.
  • Contact lens examinations are an additional service. Fees, services and testing will vary if you opt to pursue contact lenses. (Please view our Contact Lens Informed Consent form for more information.)
  • If you need eyeglasses, you can select your frames and lenses right in our in-house Optical department after your exam.

You will be asked to complete patient forms during your first visit; you can download and read those forms in advance below. You can complete our new patient form before your first visit and bring to your appointment.

Advanced Family Eye Care believes in providing the best value for you and your family. We have products and pricing that aims to accommodate all budgets and our office participates in most insurance programs. For patients who have little or no vision insurance coverage, flexible payment programs may be arranged. To learn more about our payment options, please call our office directly.

Our office accepts cash, check, and major debit and credit cards including MasterCard, Visa, Discover, as well as Care Credit.

Care Credit

Advanced Family Eye Care works with many insurance carriers and payment plans. To see if we work with your insurance carrier or vision care plan, please see the below information or call our office for assistance. Our friendly and informed staff will be happy to answer your questions.

At the time of your visit, we request that you kindly help us by providing all vision and medical forms, insurance cards, and other necessary information. While we will bill your insurance for all covered services, the patient is ultimately responsible for the remaining balance unpaid by insurance.
